Notes fail when the job cannot stop
Note-taking advice assumes a pause. A meeting can pause. A walk, a commute, a kitchen, and a conversation in a hallway often cannot. The note is needed most when stopping would drop the other thing you are doing.
The second failure is polish. People open an app and try to write the note they would be proud to reread. That standard is why the note never starts. A rough sentence stored now beats a clean paragraph that stays in your head.
